Undergraduate Medical Education

Medical undergraduate education in Spain starts after getting Spanish Baccalaureate (Bachillerato) degree and is subject to university Entrance Exam called Pruebas de Acceso a la Universidad or Selectividad. 

There are many universities in Spain which offer bachelor degree program in medicine mostly taught in Spanish. Directory of Medical Colleges of Spain

Faculty of medicine of Autonomous University of Madrid offers 6 year bachelor degree program which is divided as follows:

  • Basic medical sciences (1st and 2nd year)
  • Clinical medical sciences (3rd-5th year)
  • Clinical rotations and project at the end of year (6th year)

Spanish Medical Residency

In Spain there are currently 47 recognized medical specialties comprising 4-5 year of training. Entry into Medical residency program is possible after passing MIR (medicao interno residente sistema) exam conducted by AEMIR. Unfortunately annual quota for admission of Foreign medical/dental graduates into Residency is only 4%. This quota is applicable to those Non EU graduates who are either students in Spain or are visitors/ on visit visa. Others EU, EEA, Swiss and those Non EU graduates who have temporary or permanent residence and work permit in Spain this quota isn’t applicable for them. Call for Spanish medical residency entrance exam (MIR) is usually in the last quarter of the year. Foreign medical and dental graduates are required to complete all required documentation before date for submission. (see the procedure section below).

License/Registration

To get License and work as medical practitioner independently or in hospital in Spanish Kingdom, Foreign medical/dental graduates must register with the College of Physicians of the province where they are living in. All Colleges of Physicians of Spain are registered with Organization of Medical Colleges of Spain. They can find the contact details of all provincial colleges of physicians of Spain. These provincial colleges set up guidelines, documentation and membership criteria for registration procedure in Spain. 

Once a doctor or dentist fulfill the eligibility criteria he/she must follow procedure for licensing/registration mentioned below

For EU, EEA or Swiss graduates, their qualification is automatically recognized in Spain. (check procedure, documentation section below)

Non EU graduates must follow the procedure (mentioned in procedure section below) for license/ registration to practice their profession in Spain.

For Specialists who got training in Non EU Country can apply for recognition of their title by following these guidelines (also see documentation section below)

Eligibility

1. For License/ Registration

a) For EU, EEA and Switzerland graduates

  • Basic Medical/ dental graduation degree or diploma
  • Should have recognized title of doctor from Ministry of Health of Spain
  • Registration with licensing/ registration authority of home country or country of previous employment.
  • Spanish Language proficiency up to B2 level
  • Proof of residence in the province where applicant want to registered as doctor.

b) For Non EU medical or Dental graduates

They Should have:

a) For EU, EEA, Switzerland Specialists.

b) For Non EU specialists

They should meet following criteria

  • Should have approved his/her basic and Specialist medical/ dental degree according to the Spanish legislation (homologation/ equivalency from Ministry of education of Spain) See below.
  • Be enrolled/ registered with a Provincial college of Spanish doctors.
  • Have work and residence permit in Spain (if not Spanish citizen).
  • Having recognized the title of specialist in his/her field from Ministry of Health of Spain

Examination

1. For Specialists trained in Non EU country

The exam will consist of two parts

  • First part will evaluate theoretical, clinical and diagnostic skills and will consist of answering questions that will be structured with the degree of difficulty corresponding to the usual practice of a specialist. This part will be of 2 hour duration with total 75 multiple choice questions comprising 5 options with one correct out of them.
  • The second part will have the purpose to verify that the Specialist has capacity to make the most appropriate decisions, both diagnostic and therapeutic, to the specialty relating problems. This part will have a duration of 2 hours, and will consist of three practical scenarios with specific problems of the specialty, followed by a number of not less than three open-ended questions that must be answered with reasons.

Both parts consist of 100 marks each. To pass the test, each candidate should secure 50 marks in each of the part mentioned above. The test takes place once a year usually around last quarter annually. Dates and place are published in state newspapers or also here.

2. For Residency Entrance (MIR)

MIR exam is an entrance exam into any of the 47 recognized specialties in Spain. It consist of 225 multiple choice questions (MCQs) with an additional 10 reserve questions. Each question has 4 options with one correct to be chosen by the candidate.Duration of exam is 5 hour. Exam has no specific target subjects and questions can be about anything in the medicine. Marks obtained in the exam contain 90% of final merit while 10% is for previous academic record. Quota for foreign medical graduates is just 4% while candidates with disability quota is 7%. It will be necessary to obtain 35% of the average of the top 10 records to access place in chosen specialty.

There are several academies and institutions for preparation of MIR exam. i.e. CTO group, Curso MIR etc.

Fee

For MIR (residency entrance Exam) it is 30 Euro

For Licensing/ Registration it is 60-70 Euro

Salary

Salary for resident Doctor is between 1000-1500 Euro/month

Monthly Salary of Family physician is from 2000 Euro – 5000 Euro/month

Procedure

1. Homologation/Equivalency Process for Non EU graduates

If your medical/dental qualification was issued by one of the the universities from this List of Exempted Institutes from Homologation process, you do not need to follow the following procedure

All other medical graduates who want to sit for Residency Exam (MIR) they should first apply for equivalence/homologation of their medical/dental qualification in Ministry of Education, Culture and Sports of Spain. They should fill application form, collect all certified (notarized) copies of their documents and pay the fee (See documentation section) and submit those in  any public register of the Central Government, autonomous regions and some of the Local Authorities (offices and their address can be found here). Once your equivalence certificate is ready, collect it from Department of Qualifications and Recognition of Qualifications Street Torrelaguna, 58 28027 MADRID Spain.

(Note: Apply for Equivalency process as soon as possible it may take up to one year to complete it)

2. For residency entrance Exam (MIR)

a) For EU, EEA, Switzerland graduates

You do not need to follow homologation or equivalency process described above but instead you should apply for “Recognition of professional title to Ministry of Health of Spain”. It takes less time and when you get your title recognition certificate you should apply for MIR entrance exam. You must pay the fee, submit all required documents mentioned in Documentation section and apply for registration for residency exam (MIR). (check documentation section below)

b) For Non EU medical or dental graduates

First of all you should apply for Homologation process (described above) with documents mentioned below in documentation section. Once you get equivalence certificate (it may take up to 1 year for the process to be complete), then apply for residency entrance exam (MIR) with documents mentioned below before the deadline. Deadlines are published annually in official gazette.

3. For License/ Registration

a) of EU, EEA, Switzerland graduates

They should first apply to Ministry of Health, Social Policy and Equality of Spain for title recognition with documents mentioned below in documentation section. Once they get their title recognition certificate from the Ministry of Health of Spain, they should apply to their Provincial Medical College of Physician with documents mentioned below in documentation section.

(Note: Specialists should also get recognition of their Specialist title from Ministry of Health, Social Policy and Equality and submit it while applying for License/ Registration with the Provinicial Medical College)

b) of Non EU graduates

Foreign Non EU medical or dental graduates Should first of all apply for NIE number, Work permit and residence permit in Spain. Once they get these then they should apply for Homologation / equivalency certificate to Ministry of Education, Sports and Culture of Spain. It usually takes 6 months to 1 year. Once they get that they should apply for registration with their Provincial Medical College of Physician with documents mentioned below in documentation section. It’s always best to make appointment first before going for registration e.g. for Official Medical College of Barcelona you can make appointment online here.

(Note: Specialists should also get homologation/ equivalency of their Specialist title from Ministry of Education, Sports and Culture and submit it while applying for License/ Registration with the Provinicial Medical College)

4. For recognition of Non EU trained specialists in Spain

Those specialists/ consultants trained in Non EU countries should follow this procedure in order to be recognized or registered as Specialists in their field in Spain:

First of all they should start with Homologation/ equivalency process (apply online or fill application form with documents mentioned below). Once it’s complete and they get equivalency certificate. They should apply for recognition of their specialist qualification by filling the application form (see below documentation section) and sending all required documents mentioned in the application form to Ministry of Health, Social Services and Equality of Spain. (SR. MINISTRO DE SANIDAD POLITICA SOCIAL. E IGUALDAD Subdirección General de Ordenación Profesional. Paseo del Prado, 18 y 20.- 28071 MADRID).

Documentation

1. For Homologation (equivalency) process for Non EU graduates

  • Apply Online for Equivalency Procedure OR
  • Fill this Application Form for Professional Recognition
  • Certified copy of passport, Alien Immigration Card etc.
  • Certified copy of graduate/postgraduate medical qualification (MBBS/MD etc) for which recognition/ equivalence is required
  • Certified copy of the academic certificates, transcript, duration, the curriculum followed, the subjects studied and the hourly load and the total teaching workload, also certified copy of the official translation.
  • Certificate of Spanish Language Proficiency up to B2 level mentioned in this Order.
  • Proof of payment of the fee by filling fee-submission-form-for-equivalence and fee payment in the bank or if sending from abroad, payment of the fee should be made by bank transfer to the Ministry of Education in the restricted account for collecting fees abroad: account No. 0182 2370 44 0200203771 of BBVA, located at C / Alcalá, 16, 28014-Madrid ( Spain).or . (Note: DO NOT send any original document. send only attested copies from Registration of the Ministry of Education,Culture and Sport, Delegations and sub – delegations of the Government,
Embassies and Consulates of Spain or Notary Public)

2. For EU, EEA or Switzerland graduates recognition of title by Ministry of Health of Spain

  • Duly filled and signed Application form for Recognition of title issued by EU, EEA  or Switzerland (and all documents mentioned in it should be attached)
  • Copy of national identity card , passport or equivalent documentation certifying that the person concerned is a national of one of the Member States of the European Union, European Economic Area or Switzerland.
    For third-country nationals who have acquired their qualifications from EU, EEA or Switzerland should present supporting documentation of equal treatment laid down in the relevant Directive 2005/36 / EC of 7 September 2005 may apply.
  • Copy the title or academic qualifications and, where appropriate, professionals whose
    Verification is requested
  • official certification and personalized training program conducted by the Applicant, stating:
    . Duration of studies in academic years.
    . Description of subjects taken, specifying the number of hours, credits ECTS in each of them, both theoretical and practical.
  • Certificate of registration/ license to practice stating professional title of the candidate from country of Origin EU, EEA or Switzerland
  • Good standing certificate from competent authority of the Member State of origin or where the applicant is established to be a professional, not more than 3 months old from the date of issue.
  • Certificate of employment of one year full time or part time within last 10 years of same duration issued by the competent authority in the State which issued the title of the profession.
  • Those doctors, dentists or health professionals whose training do not meet all the Training requirements mentioned in Chapter III of Title III of Directive 2005/36 / EC should provide certificate stating that they completed 3 consecutive year of training preceding 5 year of issuance of their profession title.
  • Certificate of Homologation from Ministry of Education, Culture and Sport should be submitted if applying for Specialist recognition
  • Training certificates issued in third countries may only be recognized in accordance with the procedure laid down in Directive 2005/36 / EC if they have been previously recognized in another Member State and the holder of the evidence of professional qualifications in the profession concerned. Three years in the territory of the Member State which has recognized such evidence of formal qualifications, certified by the latter

(Note: All these documents should be submitted to central services or Peripheral centers of the Ministry of Health, Social Services and Equality of Spain)

3. For License/ Registration to work as Medical or Dental Practitioner in Spain

a) For License/ Registration of EU, EEA and Switzerland graduates 

  • Medical or Dental Qualification (original and photocopy)
  • Certificate of Registration as a resident community. (NIE) (original and photocopy)
  • Passport (original and photocopy).
  • Two passport size photographs.
  • Documentation of private homes and professional correspondence. It is essential that the place of business is located in the province of Registration. You must also designate which of households declared want to appear in the public registry of doctors.
  • A certificate issued by the competent authorities of the country where you have recently worked to ensure that you are legally entitled to practice medicine and you have not been disqualified or disabled to practice. This certificate will have a maximum validity of three months from the date of issue.
  • Make payment of entry fee .Payment can be made in cash or by credit card.

b) For License/ Registration of Non EU medical or Dental graduates to work in hospital in Spanish Kingdom

  • Homologation/ equivalency certificate of the title of medical or dental graduates (original and photocopy).
  • Medical graduation degree, diploma original and photocopy
  • NIE number, Residence permit and work permit of Kingdom of Spain
  • Passport (original and photocopy
  • Two passport size photographs
  • Documentation of private homes and professional correspondence. (a document indicating your home address in Province of Provincial College of Medicine you want to get registered with)
  • A valid certificate of registration with competent health registration authority (license/ registration issuing authority) where you have worked
  • A good standing certificate from competent health registration authority (license/ registration issuing authority) where you have worked, not older than 3 months and should be dully apostle (it means your certificate should be notarized if issued from countries which is part of Hague Convention Apostille, if it’s not then it should be stamped by Ministry of Foreign Affairs of country of origin, Spanish Embassy or Consulate in state of Origin, Ministry of Foreign Affairs and Cooperation (MAEC – Legalization Section)
  • Payment of Fee by cash of credit card.

4. For Residency Entrance Exam (MIR)

a) For EU, EEA, Switzerland graduates applying for MIR exam

  • Photocopy of the passport or ID card.
  • Certified photocopy of the resolution of recognition of the Title of Graduate in medicine and surgery. (for EU, EEA and Switzerland title recognition see process above)
  • Certified photocopy of graduate medical/ dental qualification and certified copy of any relevant certificates, transcript etc.
  • Filled and signed Affidavit (Annex IV of the call
  • Proof of payment of the fee
  • Proof or certificate of Spanish language proficiency up to C1 level (for non Spanish countries candidates)

5. For specialists trained in Non EU country applying for recognition of their title in Spain

All documents must accompany official translation into Spanish and should be sent as notarized copies only.

Postgraduate Medical Education

Faculty of Medicine of University of Barcelona offers several Postgraduate Masters degree courses and PhD programs for local and foreign doctors and medical graduates. Faculty of medicine of Autonomous University of Madrid also offers some masters and PhD degree programs.

Almost all Provincial Colleges of Spain also conduct and regulate continuous medical education courses, seminars, conferences etc. e.g. College of Physician of Madrid regularly conducts courses of continuous medical education through Foundation for education and public health training

    I m dr from pakistan….i had spend 3 y in spain have done 2 levels of spenish….i was submitted my documents in spain ministry but after One and half year thy refused my documents …..i came back to pak fo my one year house job…..now i can apply again is its benificial…..spain really disheart mi..

      well yeah it happens. if you know spanish language you have experience you can apply again. you can also apply to another spanish country. there are over 40 spanish countries in the world.

    Hello, I just came across this information which I thought was interesting. I graduated from a Caribbean medical school and did my rotations in US. I’ve applied to residency in US a few times but unfortunately have not had any luck. So I’ve been looking into other countries. Although I have an MD degree, I don’t have any practicing license. Do I still qualify for this process even though I don’t have a practicing license?

      Well Practicing license from any country is a prerequisite for Europe. So I will suggest you to first license from any country. Be it even a small one. Because you will definitely need it whenever you will apply for any other country.

        Thank you for your response! Well I graduated in 2012, so I’m not sure if that’s recent enough for Qatar and UAE. What do you think? And are there any other countries that I have a chance of getting based on my qualifications? I see that you posted info on medical residencies in countries Germany and Switzerland.
